Jane’s Inner Conflict In the novel Jane Eyre, Jane is often taken forced to make life changing decisions. Her decisions not only affect her own life and those around her, but also provide a model of a modern woman who is free from the social expectations for women living in her era. From a young age, Jane was taught the mindset that she must be independent and fight for her place. She was neglected as a child while living at Gateshead and often left to fend for herself. She was frequently locked by herself in a room for hours on end and continually accused of being a liar. From fighting John Reed to leaving Rochester moments before their marriage, Jane always emerges as an individualistic figure that refuses to conform to anyone’s wishes except her own. The tough decisions she makes for herself end up benefitting her in her pursuit to break the cultural norm of women in society. Jane Eyre, though often oppressed and looked down upon by figures in authority over her, em erges as a symbol of freedom albeit facing manifold obstacles in her life. Throughout the novel, Jane proves her worth with her intellect and desire for independence. The book portrayed in the drawing represents Jane’s education and how it allows her to have a respectable job as a governess. Since she received an education at Lowood, Jane allowed for herself more opportunities to remain less dependent on others and allow herself to make her own decisions. Clarences Speech in William Shakespeares Richard III The speech I am going to be examining is from one of Shakespeares plays, Richard III. It is the part of the play where George, Duke of Clarence is explaining a prophetic nightmare he had whilst being locked in the Tower of London. Richard III was a real king of England, but had been killed in battle, around 100 years prior to when Shakespeare wrote this play. At the time of writing, in 1594, the queen was Elizabeth I, a Tudor. One of the Queens ancestors, Henry VII, had killed Richard III on Bosworth Field. Shakespeare, to make sure he was in favour with the Queen, made Richards character into a complete villain, as well as physically†¦show more content†¦This helps to build up drama and create a vivid picture in the minds of those watching the plays. This was especially important in the Elizabethan times, as stages had limited scenery and special effects. The beginning of Clarences speech is full of warnings and phrases that suggest death. He begins his speech telling Brackenbury that, in his dream, he thought he had broken from the Tower and was on a ship to Burgundy with Richard. He says Richard tempted him to walk over the deck. The word tempted is a word often used when talking of the devil as the devil tries to lure people into sin. When Clarence uses this word to describe Richards actions, it is like if Richard is being described as devil-like. He then says that he and Richard looked towards England, and recalled a thousand heavy times, during the wars of York and Lancaster, that had befallen us. A thousand is an exaggeration, but it shows that Clarence is being warned of more bad things to come. This sets the mood of the speech. It is full of warnings and sentences suggesting bad times. Clarence says Methought that Gloucester stumbled, and in falling, struck me. This is a forewarning that Richard is going to cause Clarence to suffer in the future. Importance of Critical Thinking for Students-Samples for Students

Question: How can critical thinking be learned and demonstrated by higher education students? Answer: This article is based on the description of how critical thinking can be linked with cognitive science. It provides a new and fresher perspective to the generalist specifist debate to make progress in the structure of interventions for teaching and developing critical thinking in the students. It describes the ways by which cognitive theories circumvent human tendencies toward less rational and less effortful thought. The article gives an insight how the cognitive science contribute in enhancing higher education teaching students to develop their critical thinking skills. The article described the methods of cognitive science in developing critical thinking in the students pursuing higher education. It suggests practices and principles to engage students in deeper critical thinking by examining skepticism, higher-order thinking, problem solving, metacognition and many more. This paper rejects the informal logic for critical thinking as it is too much biased and do not follow the cogn itive mechanism necessary for critical thinking. According to this article, the students can learn and demonstrate their critical thinking if it is taught with the help of cognitive science, though it is more difficult to implement the methods than the informal logic. It provides an idea, why there are limitations exist in learning of critical thinking and how it can be addressed. The paper presents an investigation under controlled laboratory condition after breaking down the components of critical thinking among the students. This paper identifies the importance of critical thinking for students pursuing higher education to make effective contribution in the global market. The first half of the article provides insight of the existing assessments and their psychometric qualities. It uses the critical thinking assessment to identify the challenges surrounding the current design and implementation of those designs. The second half on the other hand, uses the data and proposes the next generation critical thinking frame that needs to be implemented in order to help the students in learning and demonstrating in the world workplace. The article is of critical importance for the institutes that are planning to design or adopt an assessment of critical thinking. The institutes which already have an critical thinking program can also use this article to develop their current program for a better result and outcome. Contexts, structural features, item formats, task types and accessibility are discussed in the arti cle while designing the critical thinking of next generation. Advantages of the proposed framework is also identified and described in the article with assessment consideration. It clearly shows the complications of designing a new and better critical thinking assessment and demands the collaboration between domain experts, measurement experts, faculty members, assessment developers and institutions. An absolute need of coordination is required in this process of assessment development which is clearly identified in the article. From the above discussion, it can be concluded that both the article focuses on the next generation critical thinking approach that needs to be implemented in the higher educational institutions in order to develop the critical thinking of the students. It is crucial, as the students need a more critical thinking in the workplace and the present approaches has many loop holes that needs to be avoided. The first article can be considered more effective as it focuses on using the cognitive science to increase the efficiency of the present approaches. Analysis Of Chasing Amy Essay Chasing Amy is a movie that uncovers the hardships we find within the confusion of love and sex in our lives. Sometimes the line between love and sex seems almost invisible, but the differences and complications of understanding that are quite clear in this movie. The issues presented in Chasing Amy are directly related to the discussions of â€Å"Politics of Sexuality† as well as everyday life in our culture. Our culture, in contrast to Ancient Greece, uses sexuality to characterize people by their sexual behaviors. Other cultures view sex as a raw pleasure activity while our society has elevated it to the expression of a person’s identity and moral code. Chasing Amy contributes to this view of society as the whole movie centers around sexual beings and the influence of sex on their lives. Chasing Amy is an account of people’s fears about sex and relationships as well as the inhibitions and insecurities that are attached to them. Therefore, the best way to understand the movie’’ message is to analyze the sexual identity of the characters in it. The characters are used to portray the stereotypically viewed male and female as well as the exceptions. Some characters show a combination of both views, while others are the epitome of a sexually comfortable, confident person. The combination of different experiences, views, and beliefs in the characters allows for many confrontational debates on what sex should mean in today’s society and in our lives. The first character we meet represents a stereotypical male of today’s society, Banky. He is portrayed as an ignorant homophobe who is quick to make a joke of sexual comment. Banky takes part in all the typical male fetish behaviors such as enjoying a collection of â€Å"stroke† magazines, fantasizing about Catholic School girls, watching lesbians together, and making derogatory statements about others’ sexual orientation. Banky comments to Alyssa that, â€Å"Everyone needs *censored*. † This statement could have the typical misogynist meaning of male dominance and female submission to them in every way. It also could be a subconscious truth that Banky â€Å"needs *censored*† because he is homosexual and uses this behavior as a front in order to over it up. This position, that he is in love with Holden, is supported throughout the movie but Banky is too uncomfortable with his sexuality to admit the truth. Banky’s behavior masking his homosexuality is proof of what we repeated many times in class that we question and criticize others because it cause us to question our own reality. He was homophobic because he was uncomfortable with not fitting society’s heterosexual category. Holden is Banky’s best friend but does not have the same outlook as he. Holden seems to be an open-mind individual yet he views Alyssa as a slut when he becomes aware of her past exotic, sexual experiences, such as multiple partners. This is the typical treatment of women who are sexually experienced with men who are too immature to deal with it. Holden cannot continue their relationship as he judges Alyssa who is more experienced than he. He labels her as a slut to compensate for his feelings of inadequacy. Holden pays too close attention and gives too much significance to sex in his relationship with Alyssa. He allows his masculine pride and insecurity about his sexuality to ruin his relationship with Alyssa, which was full of love and potential. Silent Bob gives us a view of Holden in the future, wiser because of his experience with Alyssa. The first view we are given is cold, quiet disposition that makes him appear as a hard man. Yet we eventually see a more feminine side to him as his friend comments that Bob is a â€Å"softy† because he cries while listening to the Barbara Streisand song, â€Å"You Don’t Bring Me Flowers. Silent Bob, which had the same predicament as Holden, grew because of his loss and is no longer afraid of his feelings of inadequacy. He is now a better person and on a higher level of understanding his sexuality, capable of admitting his shortcomings and uncertainties. Hooper is an interesting mediating character as he is homosexual but acts heterosexual for his public image as a strong Black social figure. The stereotypical view of a homosexual would not be good role model because he would be viewed as weak and perhaps not masculine. Fight Club Analysis EssayAlyssa found her moral standards based on her inner self instead of society and she was criticized because of this. Sexuality is rewarded or punished in society because of this. Sexuality is rewarded or punished in society because it exists in a context, as for Alyssa with a loss of reputation as respectable woman (Rubin). Her experienced past is unexpected and not accepted while men are expected to have this experience to be real men. This is the reason why Holden is unable to deal with his girlfriend having more diverse sexual experiences than he. There are also some similarities with the way lesbians treat Alyssa and the rest of society. Alyssa is evaluated by her friends as non-gender because she is now in love with the dreaded male. These seemingly feminist women are supposed to want the best for her regardless if she is happy with a man but they are too involved in the stereotype to do this. Alyssa follows Rich’s law that sexual identity can potentially change throughout life yet Alyssa is compelled by both homosexuality and heterosexuality to follow society’s norms of each (Compulsory Heterosexuality and Lesbian Existence). Alyssa’s continuum causes her discomfort as well because as people change, their whole world is â€Å"put into upheaval† and things she did once did are not of the same person as she is now or what people expect. Lesbians put the same pressure on Alyssa to conform to their group norms as society does for people to be heterosexual. We see the movie’s position on other connected issues about sexuality as well. We see some of the stereotypical definitions disregarded in this movie such as the â€Å"accepted† definitions for â€Å"*censored*† and â€Å"virginity. † This ties into the lacking of a pluralistic ethic and the concept of a benign sexual variation. Both these theories surround upon the idea that there is only one context to have sex and that we should follow this narrow intolerance of variation. Banky is unable to undaunted the alternate meaning of â€Å"*censored*† when designated toward two women. This is fitting to his ignorant character that he is unable to see past the stereotypical view of sex. Holden also is puzzled by the unaccepted definition of loss of virginity not including intercourse. In addition, the movie shows the acceptance of some sex while other kinds of sex are less accepted. This hierarchical evaluation of sex acts (Rubin) is the cause of Alyssa’s problem with Holden. Her being a lesbian is acceptable but the thought of her with two guys is unbearable. Lorde’s ideas are also touched upon where sex is better when it is with someone you love. Holden believes his solution is accepted because he loves the two people involved. Silent Bob also says that Amy was not looking for an experience but for love with someone special. There were some points that made me truly think about myself and the unquestioned path I have taken. Alyssa does not limit herself to men or women in order to find her soul mate. I think this is important because you love someone because of who they are, not sex, color, or looks. If you limit yourself by these things you lose out. It is important to expand your horizons and leave other opportunities open to mind. I also liked Alyssa’s idea of needing a map to find who we are in the world. We all need to explore and discover what we truly want from life. Holden’s last comment is that because of his experience with Alyssa he finally had something personal to say. He grew as a person because of his new experiences with her and actually gained an experience he did not know he wanted.

Aurora Borealis AURORA BOREALIS Imagine a cold October night, you walk out to your car and something catches your eye. You step further into the street be it busy or not, and you begin to turn in 360 circles. For what your eyes behold is far from anything you have ever seen. A circle of green and blue swirl through the night sky, I could best describe this as two ballroom dancers doing a Waltz through the night sky. They flow, they glide so gracefully that they capture you and keep you entranced for a very long time. This my first experience viewing the amazing Aurora Borealis, it did not matter that people were watching, or that the roads were iced over and that I could not stand on my own two feet very steadily. For as unbalanced on my feet I might have felt, I felt very balanced watching these ?ballroom dancers? in the night sky. October 1996, Fairbanks, Alaska. As enthralled as I was with aurora borealis, so were many other cultures. In Middle-Age Europe the Aurora's were thought to be the reflections of heavenly warriors. As a kind of posthumous reward the soldiers that gave their lives for their King and Country were allowed to battle on the skies forever. The northern lights were the breath of these soldiers as they resumed their fight in the sky. The Scots saw the lights as ?Merry Dancers?, while the Swedes thought the lights performed folk dances and polkas. The power of the auroras is said to be fruitful for the earth, they draw animals to certain areas so that they are plentiful for hunters. The Scandinavian people would say the lights are sunshine reflecting off of the herring in the northern sea. A welcome omen for the fishermen to reel in large amounts of fish. According to folklore tradition shared by some Alaskan Eskimos, the northern lights are the souls from those whom died from massive blood loss. Be it from childbirth, murder, or suicide. The spirits from these bodies would play a game similar to that of soccer in the night skies, but they would use either the head of a walrus or of a human. It would seem as though you could hear the chattering as the jaws thrust toward the players. There are reports that the auroras have the ability to heal, however; their ability to bring on ailments is far more founded. Researchers from the USSR published results that stated that people are more likely to have fatal heart attacks when the auroras are at their strongest point. The myth and mystery of aurora borealis is so widespread. It encompasses so many countries that it is impossible to share all their thoughts. My favorite folklore is that of the Eskimos of the lower Yukon River in Alaska. They would describe the Aurora's as the dancing souls of their favorite animals: deer, seals, salmon, and beluga whales. Under the spell of the Aurora's the Universe comes to life and people will be drawn to their loveliness, no matter what the myths or folklore share, the Aurora's are magical and spell binding for all whom have the opportunity to experience them. The Aurora's are never absent from the earth, they are just often not seen. Viewing them is easy if you are fortunate enough to live in the favored locations. The Aurora's are prominent in the Southern and Northern Hemispheres and spend most of their time around the edges of the earth where they are said to dance solely for the penguins and the polar bears. However, due to the tilting towards the sun and irregularities in the earth's magnetic field, the aurora is 20-30% brighter in the northern hemisphere. ?Where the auroras are most common people tend to be scarce.? Examples of these areas that have phenomenal views of the auroras would include; Goose Bay, LaRonge, and Yellowknife, Canada; Barrow, Alaska; Tromso, Norway; and parts of Russia. Market segmentation is a challenge by marketers to identify group of customers with similar attributes and then target the product according to their different needs. These segments which are made as a result of market segmentation process, are then refined and one or several segments are selected to target a particular product or market offering. The process of selecting appropriate group of customers for a particular market offering is a part of planning before the product is initially launched in the market. The overall marketing efforts are the post consequences of this process or in other way these market efforts are based on attributes possessed by the customer. Each set of customers is targeted according to their attributes. Therefore it is of crucial importance for marketers to identify these attributes of customers that can be satisfied by the product offered in the market. We included these products in our survey to identify customers acceptance of a product that is linked to a particular segment by another segment that is not targeted by that product. Problem statement To study the effects of linking market offerings to specific customer segments on its acceptance by other customer segments. In this problem statement well study the effect of linking market offerings to specific customer segments as an independent variable. We will be including market offerings that are linked to a particular market segment and will study its effect by identifying the acceptance of these products by other segments. Therefore, the acceptance of market offerings that are linked to a particular segment by another segment will be dependent variable. Hypotheses For conducting this research we have created the following hypotheses: H1: Linking a market offering to a particular customer segment based on age does not have any effect on its acceptance by another segment based on age. H2: Linking a market offering to a particular customer segment based on gender does not have any effect on its acceptance by another segment based on gender. H3: Linking a market offering to a particular customer segment based on geographic location does not have any effect on its acceptance by another segment based on geographic location. Outline of the study Initially this study included the introduction to our research. In first chapter we tried to elaborate on our research topic. We discussed the issues that lead us towards our research problem. Furthermore we motioned our research problem statement along with the variables well study from our research problem. To test the variables we have formulated our hypotheses that well check in our result section.   Ã‚  Ã‚  Ã‚  Ã‚   Gene Therapy Gene Therapy is the insertion of a gene or genes into cells in order to provide a new set of instructions to those cells. Gene insertion can be used to correct an inherited genetic defect which is causing disease, to counter or correct the effects of a genetic mutation, or even to program a cell for an entirely new function or property. Genes are composed of molecules of deoxyribonucleic acid, or DNA (see Nucleic Acids), and are usually located in the nucleus of cells. The instructions that direct the development of an organism are encoded within the genes. Some diseases such as cystic fibrosis are caused by an inherited genetic defect. Other diseases are caused by a miscoding of a gene so that the instructions it contains are disrupted or changed. Genetic miscoding occurs when the DNA in a cell is being duplicated during cell growth and division (somatic mutation) and is common when a cell becomes cancerous. Clinical use of gene therapy began on September 14, 1990, at the National I nstitute of Health in Bethesda, Maryland, when a four-year-old girl received gene therapy treatment for adenosine deaminase (ADA) deficiency, an ordinarily fatal inherited disease of the immune system. Because of this genetic defect she was susceptible to recurrent life-threatening infections. Gene therapy for this patient involved using a genetically modified virus to carry a normal ADA gene into her immune cells. The inserted ADA gene then programmed the cells to produce the missing ADA enzyme, which led to normal immune function in those cells. This treatment temporarily helped her to develop resistance to infection. Gene therapy may eventually provide effective treatment for many diseases that currently have no cure. These include inherited disorders such as cystic fibrosis, muscular dystrophy, and juvenile diabetes. In addition, gene therapy can also be used to treat many disorders that are not inherited, since gene insertion can also program a cell for an entirely new function . Several new therapies for non-genetic disorders are currently being tested.

Lucy v. Zehmer Case Brief Essay

I. Statement of the Facts Zehmer owned a Farm that Lucy had made several offers to purchase, all of which Zehmer rejected. Lucy met Zehmer in the latter’s restaurant one evening. After drinking, they had a substantial discussion about the sale of the farm. Lucy made an offer of $50,000. Zehmer drafted up Lucy a contract specifying the land, the amount, title satisfactory to buyer. Lucy took the written agreement and offered $50,000 to Zehmer who refused to abide to the written agreement. Lucy filed a lawsuit against Zehmer to compel him to transfer the title of the farm to him for $50,000. Zehmer supported that the writing was prepared as a bluff, the whole matter was a joke, no binding contract was made, and at the time he made the offer he was â€Å"was as high as Georgia pine†. The trial court ruled in favor of Zehmer. Lucy appealed to the Supreme Court of Appeals of Virginia. II. Issue Does the offer made by Zehmer – supposedly as a joke – constitute a valid contract? III. Rule of Law â€Å"The first requirement for effective offer is a serious intent. Not determined by subjective intentions, beliefs and assumptions of the offeror. Rather it is determined by what a reasonable person in the offeree’s position would conclude that the offreror’s words and actions meant.† Offers made in jest, obvious anger, or undue excitement do not count and the acceptance of them does not create an agreement. IV. Analysis In his defense, Zehmer claimed that the offer to sell his farm to Lucy was made as a joke and at the time he made it they were both drinking and he was not in a state of sobriety. The court judged Zehmer’s argument about his mental state inconsistent due to the fact he was able to provide great detail what was said and done that night in his testimony. The court found that the contract was discussed for forty minutes, rewritten to include Mrs. Zehnmer, and she did sign it. The discussion of what to be included in the sale and the provisions of it made a serious transaction. The delivery of the written contract shows a good faith offer and a good faith acceptance.

No other chocolate can ever compare with the great tasting M & M's. Smarties are larger than M & M's but unfortunately, they just don't taste as good. Many believe that the bigger the size of the chocolate, the better it will taste. This is not always the case, M & M's are much smaller than Smarties and they taste better than they do. A major difference between the both of them is the shell coating the chocolate. The shell of a smartie is quite hard and every hard to bite into, but the M & m shell is perfectly right, not too hard and not too soft. Another difference with the shell is that when you place the Smartie in your mouth and slowly absorb the colouring until u reach the shell, it tastes disgusting. Compared to the shell of an M & M your mouth begins to water when the shell is slowly crunched in between your teeth. The Smartie will never be able to have a tastier shell than M & M's. The chocolate within that scrumptious shell is magnificent. It just melts in your mouth, leaving a nice taste that excites your taste buds. The chocolate used for making Smarties is very bland, and it tastes like cheap chocolate Easter eggs. When you decide to indulge into a Smartie be shore that you don not hold then in your hands, because they will melt in your hands, and not in your mouth. When indulging in an M & M they just melt in your mouth, and not in your hands. When eating the other chocolate you must be near a water source so the colour does not stain your skin. They are very dangerous chocolates because they can also stain your clothes. Many mothers out there would rather give her child a M & m's which does not stain anything than a Smartie which would make them have more washing. See when you buy a packet of Smarties, and you look at the packaging, there is nothing that appeals to the average human being. When observing the packaging of the M & M's, people are automatically draw to it. The packet has those adorable little cartoon M & M's that convince you to buy their product. Now you don't see Smarties being the official candy of the millennium that's because M & M's are. Due to there scrumptious taste, and mostly because 'MM' is the roman numerals for 2000. It's like it was fate or something that M & M's are going to be the official candy of the millennium. Now I know, I'm going to buy M & M's from now on. That's the common reply from many who have stepped higher in life, when they begin to chose M & M's over Smarties, for various reasons that have already been explained.

How to Make Frozen Bubbles on Dry Ice Dry ice is the solid form of carbon dioxide. You can use dry ice to freeze bubbles solid so that you can pick them up and examine them closely. You can use this project to demonstrate several scientific principles, such as density, interference, semipermeability, and diffusion. Materials Needed Bubble Solution (from the store or make your own)Dry IceGloves (for handling the dry ice)Glass Box or Cardboard Box Procedure Using gloves to protect your hands, place a chunk of dry ice in the bottom of glass bowl or cardboard box. Marriage has been an important part of life since the Biblical times. It usually consists of the union of a man and a woman, but for gays and lesbians, it is the union of two men and two women. On June 26, 2015, the US Supreme Court ruled that the US Constitution guarantees the right for same-sex couples to marry. Many conservatives are completely against gay marriage and they have stated that they will fight to have the Supreme Court ruling overturned. Legalizing gay marriage is a very difficult issue for much of society to accept. Some of the controversies with gay marriage are religious beliefs, equal rights, legal ramifications, a fight for acceptance and the freedom to practice their lifestyles openly while society can contend that gay marriage is a bad idea for the sake of procreation, because the consequences far outweigh the benefits. We can concede that the emotional stigma associated with gay marriage makes it more difficult for children to fit societal standards. First of all, there are many reasons that are given to refute why gay and lesbian marriages should be a part of our society. Marriage in this country is defined as the union between a man and a woman, but over the past couple of years there has been much controversy over the true meaning of marriage. For the vast majority of professing Christians today believe that the Bible teaches that monogamy is the only acceptable form of marriage and that gay marriage violates God's commandments concerning marriage. Savage Inequalities by Jonathan Kozol :: Education Poverty Public Schools

Savage Inequalities, written by Jonathan Kozol, shows his two-year investigation into the neighborhoods and schools of the privileged and disadvantaged. Kozol shows disparities in educational expenditures between suburban and urban schools. He also shows how this matter affects children that have few or no books at all and are located in bad neighborhoods. You can draw conclusions about the urban schools in comparison to the suburban ones and it would be completely correct. The differences between a quality education and different races are analyzed. Kozol even goes as far as suggesting that suburban schools have better use for their money because the children's futures are more secure in a suburban setting. He thinks that each child should receive as much as they need in order to be equal with everyone else. If children in Detroit have greater needs than a student in Ann Arbor, then the students in Detroit should receive a greater amount of money. My perception was changed completely after reading this book, I never knew that so many schools were situated in the ghettos and were so badly overcrowded or only had two toilets working for about 1000 students, and no toilet paper. What really upsets me is the fact that within the exact same city limits, there are schools situated in the suburbs which average 20 per classroom and have enough supplies and computers for every child to receive one as their own. Of course the majority of these suburban schools are dominantly white and the urban schools hold the minorities. The dropout rates that are listed in the book are ridiculous. Most of the children drop out in secondary school and never receive a proper education because of the lack of supplies or lack of teachers' interests. The majority of the kids are black or Hispanic in the poor schools and the suburban schools hold the upper-class white children and the occasional Asian or Japanese children who are in the gifted classe s. The small population of blacks and Hispanics that go to the schools are placed into the "special" classrooms and their "mental retardations" can be blamed for their placements. The majority of these students are not mental and they belonged in a regular classroom among whites and Asians. Kozol argues that the system is separate and unequal and he builds upon his hypothesis until it becomes credible.

Nurses’ Work Hours

I have been a staff nurse in the emergency room for fourteen years. I have worked a variety of 8, 10, 12, and even 16 hour shifts. I currently am working 8 and 12-hour shifts on nights. Previously, I also worked some 12-hour day shifts. I personally have been struggling with working the 12-hour shifts. My commute to work is 1 hour one way and I have fallen asleep several times driving home from work. The last four hours of my shift I experience great fatigue and even have had trouble staying awake. I have come close to having medication errors and feel that my quality of care is less during the last four hours. When working 12-hour shifts I average around 5-6 hours of sleep before returning to do another 12-hour shift. During an interview with another colleague also working 12-hour shifts she states she also experiences feeling very tired and less alert. She actually admits to having a medication error that she feels was associated with the long hours and fatigue. With these concerns I ask, â€Å"Are nurses work hours a concern for nurses and patient safety†? Introduction As demands for flexible work hours and a balance between home and work life have increased for nurses, twelve hour shifts are more common. The nursing shortage has also contributed to nurses’ working longer hours to cover shifts. Nurses must remain alert to provide safe care and prevent errors in medications and procedures. Nurse work hours are a concern to me regarding patient safety. I am also concerned with the health risk of nurses working long hours so I decided to do a search on how long hours affect nurses and their patients. I searched evidenced based research articles available from a variety of trustworthy healthcare sites like CINHAL, ANA, and Nursing Journals. I organized this literature review in three categories. These categories are: Positive and negative effects of long hours, effects of long hours on patient safety, and if there are any health effects on the nurse working 12-hour shifts. Literature Review Positive and Negative Effects of Long Hours There are both advantages and disadvantages of working 12 hour shifts (Ede, Davis, & Sirois, Circadian, 2007). One advantage is working less days during the week, which is desirable of most nurses. It is also easier to have all the shifts covered because you do not have to hire as many nurses (Circadian, 2007). In a qualitative study by Richardson, Turnock, Harris, Finley, & Carson (2007) the purpose was to examine the impact and implications of 12-hour shifts on critical care staff. Two groups and questionnaires with critical care staff from three critical care units were reviewed. Positive effects were found with planning and prioritizing care, improved relationships with parents/relatives, good quality time off work and ease of travelling to work. Less favorable effects were with caring for patients in isolation and the impact on staff motivation and tiredness. Acceptable patterns of work were suggested like no more than 2-3 consecutive shifts should be worked and rest periods between shifts. The survey concluded that most of the nurses that responded wanted to continue with 12-hour shifts. Systems and practices need to be developed to improve on the negative effects of working 12-hour shifts. In one of my interviews the nurse agreed that she enjoys 12-hour shifts because she has more days off during the week and feels she has more time with her family, although I personally disagree. She also states it is easier to provide day care for her children. Disadvantages of longer work hours according to Circadian (2007) included that it is harder to cover absences, limited family and social time during working days, more pay lost when a day is missed and increased percentage of night shifts. Longer work hours have also been associated with increased tiredness, less sleeping hours, driver fatigue returning home, and increased risk of errors or near errors ( Richardson et al. , 2007); (Scott, 2006); (Scott, 2010); (Chen, 2011). I have no time with my family on the days I work 12 hours. Even though I have more days off I feel I am spending at least one day recovering after working a long shift. I personally would rather have interaction with my family daily especially since the kids are in school. A colleague I interviewed also agreed that when working 12 hour shifts she had limited time with her family. Effects on Long Hours on Patient Safety As a result of nursing shortage hospital staff nurses are working longer hours with few breaks (Chen et al. 2011). In one quantitative study by Scott, Rogers, Hwang, & Zhang (2006) they randomly selected 1148 critical care nurses and sent them a demographical questionnaire to fill and return related to medical errors and the hours they worked. The objective of this article was to describe the work patterns of critical care nurses, determine if there was a relationship between the occurrence of errors and the hours worked by the nurses, and explore whether these work ho urs had adverse effects on nurses’ vigilance. The 502 respondents consistently worked longer than scheduled and for extended periods. Longer work duration increased the risk errors and near risk errors and decreased nurses’ vigilance. The findings supported the Institute of Medicine recommendations to minimize the use of 12 hour shifts and to limit nurses’ work hours to no more than 12 consecutive hours during a 24 hour period (IOM, 2006). Although the findings note that 12 hour shifts can have negative effects, most participants wanted to continue working them. Nurses are responsible for the safety of their patients. Nurses must remain alert to provide safe care, recognize discrete changes in patient conditions, and intercept potentially dangerous errors in medication and procedural orders (Keller, 2009). Nurses’ work hours are a concern given the expectation of their sustained vigilance to maintain the well-being of patients. The 12 hour shifts worked by many nurses are associated with reduced sleep times, difficulties staying awake, frequent overtime, and significant risk for error (Rogers, Hwang, Scott, Aiken & Dings, 2004)( Scott, Rogers, Hwang, & Zhang, 2006). Even though 12 hour shifts may be preferred by many nurses, studies indicate that extended shifts worked by hospital staff nurses are associated with higher risk of errors. Long hours coupled with insufficient sleep and fatigue is even risker (Scott et al, 2010). More than two-thirds of 895 hospital staff nurses reported â€Å"struggling to stay awake on duty† at least once during a 28-day data gathering period (Rogers, Hwang, Scott, & Dinges, 2003). Nurses reported fighting sleep about once every five shifts (2,258 out of 11,218 shifts). Drowsiness was not confined to the night shift; more than half of these episodes occurred between 6 A. M. and midnight. Nurses who reported shorter sleep durations were more likely to struggle to stay awake, fall asleep, and make errors while on duty (Scott et al, 2010). Effects on Nurses Working Long Hours Studies examing the health and safety consequences of nurses themselves are beginning to shed some doubt about the wisdom of continuing to use the twelve hour work schedule. According to one study, adverse health and safety outcomes of extended work hours include increased rates of musculoskeletal disorders, needlestick injuries, motor vehicle accidents, and inadequate sleep (Greiger-Brown, & Trinkoff, 2010). All of these can be attributed to reduced vigilant attention, fatigue, and decreased neuromuscular fine motor control associated with sleep deficiency. In an interview with one colleague she states that her body, â€Å"hurts all over†, when working a twelve our shift. I have fallen asleep driving before which could of resulted in injury of myself or someone else. Lengthening of the shift duration from 8 to 12 hours significantly restricts the opportunity for sleep and produces sleep deficiency (Smith et al. , 1998). Working without adequate sleep between shifts can lead to negative chronic health effects including Cardiovascular Disease, metabolic syndrome, diabetes, obesity, decreased immune function, and increased cancer risk (IOM, 2006). In a quantitative study by Chen, Davis, Pan, & Daraiseh (2011) a total 145 nurses wore monitors for one 12-hour day shift to record heart rate (HR) and work pace(WP), which were used to calculate energy expenditure(EE). The purpose of this study was to determine whether hospital nurses are experiencing physiological strain at work by examining their physiological and behavioral response patterns over 12-hour shifts. The EE and HR data presented in the study revealed a moderate physiological strain experienced by 12-hour shift nurses, regardless of their slowed paces during the last four hours. The study noted that although work pace slowed during the last 4 hours, the nurses’ heart rate continued to be elevated, which could lead to cardiovascular disorder. Moreover, inadequate work break and sleep, family care-giving responsibility and aging presented challenges that may have prohibited nurses from full recovery and potentially exacerbated the negative impacts of the 12 –hour shifts. Overall their results relates to EE and HR suggest that nursing workload of 12-hour has a negative physiological impact on nurses. Policy Implications Although there are negative effects noted in working twelve hour shifts, I don’t think it is going anywhere due to the fact that nurses love them, and the increase in nursing shortage. Keeping this in mind instead of focusing on stopping twelve hour shifts, there needs to be interventions on how to improve the 12 hour shifts to provide safer patient care and prevent health problems in nurses. The preliminary policy implications I have discovered is the possibility of limiting how many twelve hour shifts a nurse should work in a row, mandating breaks while working twelve hour shifts, allowing the night shift to nap, regulate how many hours a nurse can volunteer to work, and allow the older nurses to have an option between 8 and 12-hour shifts. In my institution there is no limit on how many hours a nurse can voluntarily sign up for as long as it is approved if it puts the nurse in overtime. Our breaks are not mandated, if we get one great, and if not we can get paid for not getting a break. Most of the time, our breaks are interrupted. Regarding naps, physicians on the night shift are allowed to sleep if there are no patients, but there is no policy stating the nurse can sleep. Some nurses actually work 24 hour shifts, but are allowed to sleep at night if they are not busy. There are times though that they are busy the whole 24 hours. In one interview a nurse stated that if she lays her head down and naps for about 20 minutes she feels much more alert. I have also tried taking a nap at work when there were no patients in the ER, and I did feel so much more alert, especially driving home. It is therefore wise to consider limiting shift length and hours worked per week per IOM recommendations. After reviewing the literature it is clear that the main problem is nurses not getting enough sleep between shifts which causes fatigue. In a study of nurses’ sleep habits, Geiger-Brown (2010) found that 58 percent averaged only 5. 5 hours of sleep. When they work three or four 12-plus hours a day, they are also unable to easily reestablish a â€Å"consistent sleep schedule†. In one research article it noted that allowing the nurses to nap, especially the night nurses showed improvement in alertness and feeling less fatigued and ultimately showed a reduction in errors or near errors (Fallis, McMillan, Edwards, 2011). It also noted that anagement needs consider to mandating that the 12 hour plus nurses are taking full uninterrupted breaks which also improves alertness. Most nurses are not taking full breaks and stay longer than the scheduled 12 hours which reduces their sleep and recovery time between shifts (Greiger-Brown, & Trinkoff, 2010). In a recent review of studies between 1970 and 1998, 12 hour shifts nurses were fatigued in 5 of 7 studies, and of 10 studies measuring performance, 4 were negative, and 6 were neutral; none showed positive effects. In this review, laboratory studies showed deteriorated performance; but field studies found no difference between 8 and 12 hours. Recent studies with stronger designs and methods have increased the evidence that questions the safety of 12-hour shifts. More recent studies as mentioned earlier demonstrate an increase in patient care errors when nurses work 12 hour shifts compared to 8 hours. The research question that has been uncovered related to nurses’ long work hours is if the long hours affect the health of the nurse and patient safety? In my institution other colleagues have stated that they feel that when working 12 hours or longer more than two days in a row really decreases their quality of care because they are tired. I feel more studies need to be done on the impact of working hours and the nurses’ health along with more evidence regarding patient safety. One ethical implication researching effects of 12-hour shifts is that the managers do not want to take away 12-hour shifts due to nursing shortage because they do not have to hire as many nurses to cover shifts. Another issue is that most nurses enjoy the twelve hour shifts even though they know they become really tired because they enjoy having more days off during the week. Managers may fear that nurses will leave to find jobs that offer 12-hour shifts. I feel this is ethically a problem with colleagues and coalitions because they are not looking at the negative effects on the nurses and the patients. With research it has become obvious that working longer hours can effect both the health of the nurse and safety of the patients yet our facility along with many others have not come up with adequate resolutions to the problem. Conclusion Twelve hour shifts contribute to flexible patterns of work, but the effects of delivery of direct care and staff fatigue are important topics for deeper examination. More recent studies as mentioned earlier demonstrate an increase in patient care errors when nurses work 12 hour shifts compared to 8 hours. Although 12 hour shifts are popular, evidence shows us that extended working hours, and working while fatigued and sleep deprived reduce vigilance and impair physical/behavioral functioning. The result has damaging effects on patient safety, quality of care provided and the health of the nurse. I believe it is the responsibility of the nurse to recognize the risks of working longer hours to keep the patient safe and to also maintain their own health. I used the Patterns of Knowing (White, 1995) to view my topic and to gather information on this literature review. These patterns of knowing in nursing include empirics, esthetics, ethics, and personal knowing (White, 1995). I used empirics by gathering factual information regarding my topic. I discussed issues in regards to ethics on my topic that was mentioned in chapter 3. I included my personal knowledge and experience with working longer shift hours, and also looked at the esthetic form by putting it all together and acknowledging what the problem is. Doing this literature review I realize that working the longer hours does put a strain on my overall health, and also has potential to put my patients at risk. I will focus more on getting better sleep, and I have asked my manager about working only 8 hour shifts. I also really want to pursue gathering more information about night nurses being able to nap.
